Turning daily compliance from a scramble into a system

A skilled nursing operator replaced manual chart audits with continuous AI review across its facilities, then extended the same platform into accounts payable, finance and acquisitions.

The challenge

Compliance depended on nurse managers manually auditing charts in the EHR, so documentation gaps and reportable events surfaced late, often during state surveys, when deficiencies were already accruing penalties. Finance ran on exports and spreadsheets, disconnected from census and clinical reality.

What we deployed

Layer by layer.

  1. Ballast

Integrated the EHR, payroll and general ledger into a single model of residents, facilities, staff and spend, including governed automation for EHR screens with no API.

  2. Current

Deployed agents that review every chart daily, correlate infections across residents into outbreak episodes, and code and post vendor invoices received by email.

  3. Keel

Shipped an issue-centric compliance board, an assignments inbox for nurse managers, an AP invoice portal, and a finance workspace with projections and an acquisitions pro forma.

  4. Harbor

Moved the platform into a dedicated cloud account owned by the operator, with a signed BAA, single sign-on, role-based access and a full audit trail.

Outcomes

What changed.

  • Every chart reviewed every day, instead of sampled
  • Reportable events routed to an owner with a deadline and an audit trail
  • Vendor invoices posted to the EHR's financials straight from email intake
  • Acquisition targets modeled from raw trailing-twelve statements
